It was a hauling accident involving a Protraxion. Pinched off between the loaded rope and the pulley wheel or the cam, not sure which.

Lesson: always keep your hands off the loaded end of the haul system and away from the hauling device.

Edit: Tyler beat me to it, but it was the 16th pitch.
